Rates & Policies
Insurance:
CB Counseling Services, LCSW, PLLC is a private pay, out-of-network provider. If your insurance includes out-of-network mental health coverage, we can help you make the most of those benefits.
To facilitate your access to these benefits, we will provide the required invoices and superbills. We are also available to help you understand the details of your coverage.
Please keep in mind that we do not submit claims directly to insurance companies and cannot guarantee reimbursement. However, we are here to support you in navigating the process so you can maximize your benefits.

Cancellation Policy:
Please provide 48-hour notice of cancellations. Cancellations made within less than 48 hours and all missed appointments will be charged the full amount of the appointment. If you are running late, please call.
Payment Policy:
Credit/Debit (Mastercard, American Express, Visa, and Discover) cards are accepted for out-of-pocket services.
Payment for out-of-pocket services is due at time of appointment.

CB Counseling Services, LCSW, PLLC will store your credit card information for the purpose of guaranteeing payment for any outstanding fees. You are required to leave at least one credit card on file, which will be used to pay outstanding balances due for services.

How is it stored?
Your credit card information is stored in an encrypted, confidential, HIPAA/HITECH/PCI compliant manner.

When will my card be charged?
Unless other payment arrangements have been agreed to by both parties in writing, your card will be charged immediately following your scheduled session time. Please refer to our client contract, which outlines our cancellation policy in further detail.

Coordinated Care:​
If you would like us to coordinate care with another provider, such as your primary care physician, school, or psychiatrist we will need an authorization to release form. We will provide this to you and you can send it back to us electronically via our confidential HIPAA-compliant platform.

Electronic Communications:
For your convenience, receipts, appointment reminders, and private messages from your therapist may be sent to you through our practice software. Such information can be accessed by a password that is only known to you. If you forget your password and need to access your private health information, please contact our office.

Please notify your therapist if you decide to avoid or limit, in any way, the use of secure messaging or E-mail. Otherwise, your therapist may communicate with you via secure messaging or E-mail when necessary or appropriate. If you communicate confidential or highly private information via E-mail, your therapist will assume that you have made an informed decision and will honor your desire to communicate via e-mail. Please do not use E-mail for emergencies.
I am often not immediately available by telephone. When I am unavailable, you can leave messages on the [Your name here] voicemail, which is confidential. For non-urgent matters, if we have not returned your call within two business days, please contact us at info@CBCounselingServicesNY.com and one of my colleagues will return your call. For urgent matters please call 911 or go to your nearest Emergency Room.

Emergency Contact Information:
If you are unable to reach us and feel that it is an emergency and you cannot wait for us to return your call, call 911, or go to the nearest emergency room. If something is less urgent and we are for some reason unreachable (e.g., on vacation, etc.), please call [information here]

Professional Records:
The laws and standards of my profession require that we retain treatment records for six years.

Confidentiality:
The privacy of all communications between a client and therapist is legally protected, and we will not disclose any information about our work without written consent. However, if we believe that a client poses a threat to themselves or to someone else and may cause serious bodily harm, we are obligated to take protective measures.​​
